چکیده

More than 20 million people in Bangladesh are considered at risk of developing visceral leishmaniasis (VL). A community-based active surveillance was conducted in eight randomly selected villages in a highly endemic area of Bangladesh from 2006 to 2008. A total of 6,761 individuals living in 1,550 mud-walled houses were included in the active surveillance. Rapid rK39 dipstick tests were conducted throughout the study period to facilitate the case diagnosis. Individuals with previous or current clinical leishmaniasis were identified on the basis of the case definition of the VL elimination program. Untreated cases of suspected VL were referred to the hospital for treatment. Socioeconomic and environmental information including bed net use was also collected. In 2006, the annual incidence of clinical leishmaniasis in the study area was 141.9 cases per 10,000 population, which was significantly increased by the following year owing to community-based active surveillance for case detection and reporting. However, early case detection and early referral for treatment led to a significant decrease in incidence in 2008. This study suggests that community-based active surveillance using a simple diagnostic tool might play a role in achieving the goal of the VL elimination program.

How Far Are We from Visceral Leishmaniasis Elimination in Bangladesh? An Assessment of Epidemiological Surveillance Data

INTRODUCTION In 2005, Bangladesh, India, and Nepal joined forces to eliminate Visceral Leishmaniasis (or kala-azar) from the region by 2015. In Bangladesh the elimination target is set at less than one new case per 10,000 population per year at upazila (sub-district) level. As the deadline approaches, we review the status of the elimination initiative in this country. METHODS We collected all...

Reducing Visceral Leishmaniasis by Insecticide Impregnation of Bed-Nets, Bangladesh

The effect of insecticide-treated materials on reducing visceral leishmaniasis (VL) is disputable. In Bangladesh, we evaluated the effect of a community-based intervention with insecticide impregnation of existing bed-nets in reducing VL incidence. This intervention reduced VL by 66.5%. Widespread bed-net impregnation with slow-release insecticide may control VL in Bangladesh.

The epidemiology of visceral leishmaniasis in Bangladesh: prospects for improved control.

The parasitic disease kala-azar (visceral leishmaniasis, VL) was first described in 1824 in Jessore district, Bengal (now Bangladesh). Epidemic peaks were recorded in Bengal in the 1820s, 1860s, 1920s, and 1940s. After achieving good control of the disease during the intensive vector control efforts for malaria in the 1950s-1960s, Bangladesh experienced a VL resurgence that has lasted to the pr...

Sustaining visceral leishmaniasis elimination in Bangladesh – Could a policy brief help?

Bangladesh has made significant progress towards elimination of visceral leishmaniasis, and is on track to achieve its target of less than one case per 10,000 inhabitants in each subdistrict in 2017. As the incidence of disease falls, it is likely that the political capital and financial resources dedicated towards the elimination of visceral leishmaniasis may decrease, raising the prospect of ...

Control Measures Against Visceral Leishmaniasis in Brazil: Their Challenges

The visceral leishmaniasis (VL) has a wide geographic distribution, with 90% of its occurrence concentrated in six countries: Bangladesh, Brazil, Ethiopia, India, South Sudan and Sudan. Dogs are the main reservoir in the VL transmission, not only for their close relationship and/or coexistence with men, but also for their immunological inability in responding to the disease successfully. For th...
